How To Become A Provider In SWMBH’s Network

Substance Use Disorder Treatment and Prevention Providers

SWMBH holds all SUD contracts directly between PIHP and SUD Provider. SWMBH performs access & UM, inclusive of referrals and authorizations, for non CMH SUD services.  Substance Use Disorder (SUD) providers must complete a New SUD Provider application to join SWMBH’s provider panel. To join the SWMBH SUD panel, submit either the New SUD Net Cost Provider Application or the New SUD Provider Application, whichever is applicable to your organization.
If SWMBH determines a need for services and indicates they want to pursue a contract, the provider will be notified directly by SWMBH’s Provider Network Department. The notification will guide the next steps, including accessing the MDHHS Universal Credentialing system to apply for credentialing approval to provide services. This process requires completion of an application and submission of documentation.
The New SUD Provider application differs from the credentialing application. BOTH must be completed before services can be provided.
For more information regarding the Universal Credentialing Process, see the Universal Credentialing page. For questions about the SUD provider paneling/application process, please contact SWMBH’s Provider Network Department by emailing swmbhprovidernetwork@swmbh.org and a team member will respond to assist.
