Population Health & Integrated Healthcare

Population Health and Integrated Healthcare are interconnected concepts that aim to improve the health of entire populations while addressing health inequalities. The population health approach focuses on the wide range of determinants of health and wellbeing. Population Health emphasizes the need for a collaborative approach to whole person care and aims to improve health outcomes through preventative measures and health equity. Integrated healthcare involves the seamless integration of physical and mental health to provide person-centered care and create a more proactive and data-driven healthcare system that can effectively address the holistic needs of the population.

Southwest Michigan Behavioral Health (SWMBH) strives to be Michigan’s preeminent benefits manager and integrative healthcare partner, assuring regional health status improvements, quality, value, trust, and participant Community Mental Health Services Programs (CMHSPs) success.

 

SWMBH developed a series of eight population health reports, one for each county in the Region, to better understand the health needs and healthcare experiences of Medicaid enrollees. These reports analyze Calendar Year 2024 data and focus on key indicators such as demographics, behavioral and chronic health conditions, healthcare utilization, and pharmacy-related risk patterns. The purpose of this work is to establish a baseline that highlights disparities, emerging trends, and opportunities for targeted improvement.

The Michigan Department of Health and Human Services (MDHHS) requires Pre-paid Inpatient Health Plans (PIHPs) to participate in certain integrated health activities and performance measures each year. The reports linked below summarize SWMBH and its participant CMHSPs efforts in meeting MDHHS integrated health requirements during Fiscal Years 2024 and 2025.
